If you wish to make a Subject Access Request, please visit the website of the Hospital Trust or organisation that holds your information. Their website will provide guidance and a link to the correct online form.

If you have reached this page directly and cannot see an application form, this means you are not using the correct access link. Please return to the organisation’s website to find their Subject Access Request page and follow the link provided there.

For Hospital Trusts and authorised organisations

If you represent a Hospital Trust or other authorised organisation and were expecting to see an online application form, you will need to use your organisation’s specific access link.

If you do not have this link, please contact your SAR360 system Manager or Administrator. They can provide the correct link from the system settings so it can be added to your organisation’s website.

Using the correct link ensures that requests are securely routed to your organisation and handled in line with data protection requirements.
