Each Hospital Trust or organisation has its own unique access link. This link ensures your request is securely routed to the correct organisation and department.

If you wish to make a Subject Access Request, please visit the website of the Hospital Trust or organisation that holds your information. Their website will provide guidance and a link to the correct online application form.

If you have reached this page directly and cannot see an application form, you have not used the correct access link. Please return to the organisation's website and follow the Subject Access Request link provided there.

For Hospital Trusts and authorised organisations

If you were expecting to see an online application form, you must access it using your organisation's unique access link.

If you do not have this link, please contact your SAR360 System Manager or Administrator. They can provide the correct link from the system settings so it can be published on your organisation's website or shared with authorised users.

Using the correct access link ensures requests are securely routed to the appropriate organisation and handled in accordance with data protection requirements.
