If you are a member of the public looking to complete an online SAR application then your best bet is to visit the website of the Hospital Trust, or other organisation, that you need information from. You should be able to find a link taking you to the correct place.

If you represent a Hospital Trust, or other organisation, and were expecting to find an online application form here instead of this page then you need a link to this site which includes the correct unique client code for your organisation. If you don't have it then please contact either a Manager or Administrator of the SARFOI application system. Ask them to navigate to the settings dashboard and share the correctly formed link (including unique client code) with you. With the correct url you can create a working link from your website back to the correct page on this site.
